Commonly Used Proposal Forms
CAHNR Principal Investigators (PIs) work with Faculty Services (SPS), InCHIP, or the Center for Environmental Sciences and Engineering (CESE) to develop and submit all extramural grant requests. Specialists within these organizations will supply the PI with all necessary forms, checklists, and guidance for proposal submission (see Contact List). Faculty pursuing grants should contact their Pre-Award specialist early in the proposal process; this is ideally as soon as the grant deadline is known. Some commonly used proposal forms include the following:Internal Proposal Review Form

This form requests basic proposal information, along with key personnel and managerial approvals. The IPR form is required to be completed for all new proposals, requests for supplements, continuations, and renewals to be reviewed and approved by SPS. Save form to your PC before filling out the form.
Cost Share Form
This form is required for proposals committing cost sharing. Department approvals are required. Save the form to your computer before filling it out.
(Note: When salary is not budgeted, this form is not required for the 1% voluntary cash contribution that SPS tracks in effort.)
Request for Approval to Serve as Principal Investigator Form

Required to be completed by employee requesting approval to serve as PI or Co-PI who are normally considered ineligible and submitted to SPS for approval.

Budget Templates
Budgets should be developed in coordination with your Pre-Award Specialist. Contact them prior to developing a proposal budget.
Note: When preparing budgets, salaries for graduate assistants should be increased by 2% per year and other personnel should be increased by 5% per year. Remaining items should be increased by at least 3% per year for inflation. *
